This fits 3.5 inches - 4 inches
This was extremely comfortable, much like a breathable athletic support without the leg straps and oversized pouch. It pulls the scrotum up as I needed it away from the incision between anus and bottom of scrotum after artificial urinary sphincter surgery. My scrotum fit with room to spare - not like another I purchased. I guess the other one was using cm instead of inches when sizing. I highly recommend this one

Perfect for what I wanted to accomplish
I ordered for support. At times I have "low hangers" which very much annoy me. This helps keep the boys up and out of the way. When they are "tight" it still keep them together and lifted a bit. I initially ordered a small thinking it meant waist size. I ordered a medium this time and it's much more comfortable. I don't have a huge scrotum but the medium is definitely better. For those curious about waist size, I'm 5' 9", 160 lbs and the medium is fine and of course all of them are adjustable.

Needs a little support
Pouch is comfortable and does its job but waist band is way to short( wife sewed a four inch section on so it would fit around my hips ), and the snap that holds it on will not stay snapped, needs a much better clasping system.The above review is for the suspensory I bought from Alpha Medical, but got placed with Sport Aid that I bought from OTCRx4U. The Sport Aid is very good in every area that the first suspensory was not.

Very good product.Provides ample support with easy access for urination.I bought 1 then went right back and purchased 3 more surpensories.I recommend this product to any male needing scrotal support.
